She was preceded in death by her parents.

Usha is survived by husband, Padma Shukla; children, Arpit (Jewelie) Shukla and Monica Shukla; and siblings, Kailash (Asha) Gupta, Asha Gupta, Vimal (Sangeeta) Gupta, Pradip (Soni) Gupta, Sadhna (Rajive) Gupta, and Seema (Anuj) Goyal.

Funeral Services will be held on Wednesday, September 11, 2019, 10:30 am, at Daniels Family Funeral Services - Wyoming Chapel, 7601 Wyoming Blvd. NE, Albuquerque, NM 87109.
